Start with consequential outcomes

Choose several work types whose failure would leave a client, staff member, or owner unable to continue: a client exception, disputed completion, vendor handoff, and deadline-dependent approval. Do not start by cataloging software features or recreating a single task-specific exception map.

For each workflow, record the trigger, authoritative artifact location, ordinary owner, decision limit, exception, backup, client communication, and recovery contact. Make the missing knowledge visible

Ask a second qualified person to walk ten recent or simulated consequential items through the map. At each step, ask: Where is the authoritative record? What decision may this person make? What happens if the record conflicts or the client changes the request? Who can recover the work if the ordinary owner is not available?

“Ask the person who usually knows” is a finding, not a recovery plan. So is a folder that has no current version, owner, or explanation of a notable exception. Keep those gaps visible rather than backfilling confidence with a generic knowledge-base tool.

Improve one handoff at a time

The smallest useful improvement may be a stable request ID, a named owner on a shared inbox, a single current agreement, or an escalation record. It may also be an explicit rule that a certain judgment stays with the service owner. Do not assume the answer is an app; first make the work and the exception path legible.

This representative map does not prove it prevents knowledge loss, improves retention, or applies to regulated records. A real operations owner must replace its placeholders and decide how sensitive information should be handled.

Is a screen recording enough for a workflow handoff?

It may explain a task, but it may not identify the current artifact, authority limit, exception path, or recovery contact. Test those decisions separately.

When is the map complete?

When a second qualified person can find the authoritative record, make only the decisions within their authority, and identify the next escalation or recovery contact for the chosen workflow.
